The Testimonial Request & Formatter

Help me collect and use client testimonials.

Client: [DESCRIBE THE TRANSACTION]
Relationship: [DESCRIBE]
Where I want to use it: [WEBSITE / SOCIAL / PORTAL PROFILE]

Produce:
1. A request message that makes it easy to say yes and easy to
   write
2. Three specific prompting questions — "how was working with me"
   produces nothing usable; specific questions produce quotable
   answers
3. A follow-up if they agree but do not write
4. How to format a long testimonial into a usable pull quote
   without changing its meaning
5. Permission language for using their words and first name
6. What NOT to do: editing quotes substantively, using full names
   without consent, or implying an endorsement they did not give

Rules:
- Do not draft the testimonial for them to approve. Ask for their
  words.
- Note any jurisdiction where testimonial use in advertising is
  regulated as [CHECK LOCAL ADVERTISING RULES]
