Pre-Mortem Analysis

I'm planning to [PROJECT/DECISION/INITIATIVE].

Conduct a pre-mortem: Assume this has failed spectacularly.
Work backward to identify why.

1. What are the most likely causes of failure?
2. What warning signs would precede failure?
3. What dependencies could break?
4. What assumptions am I making that might be wrong?
5. What external factors could derail this?
6. What haven't I thought about?

For each failure mode:
- Probability (low/medium/high)
- Impact (low/medium/high)
- How to prevent or mitigate

Then: Given this analysis, what should I do differently?
