Design a system that lets people safely raise concerns.

The organisation: [SIZE, CULTURE, ANY HISTORY OF SILENCED ISSUES]

Provide:
1. The channels for raising concerns (multiple, including a way around
   the manager)
2. How to genuinely protect against retaliation (the make-or-break
   element)
3. How concerns get triaged, investigated, and closed out
4. How to build the trust that makes people actually use it
5. What leadership must do to signal that speaking up is valued

Be clear that a reporting mechanism without genuine protection and
follow-through is worse than none — it teaches people that speaking up is
futile.
