3. Any documentation gap I need to close, listed explicitly
4. The appeal letter itself
5. The strongest single argument, and confirmation that it appears
   in the first paragraph
6. What to escalate to if this appeal fails, and the deadline

Rules:
- Address their stated reason head-on rather than restating the
  original request
- No new clinical claims I have not supplied
- Professional tone — irritation reads badly on external review
