The Medication Review Framework

5. How to structure the conversation about stopping something
6. What to document
7. Follow-up and monitoring structure

Do NOT list specific drugs, doses, interactions, or
recommendations. I want the framework; the pharmacology comes from
a current reference and a pharmacist.
