The Consent Form Plain-Language Explainer

Write a plain-language companion that explains a consent form to
a patient before they sign it.

1. Explains, in one sentence each, what every section of the
   form is actually asking them to agree to
2. Sets out what the procedure involves in everyday words
3. Explains what "risks," "benefits," and "alternatives" mean
   in general terms, with [CLINICIAN TO COMPLETE] wherever a
   specific risk, figure, or alternative belongs
4. Explains that consent can be withdrawn, and how
5. Lists the questions a patient should feel free to ask before
   signing
6. Marks clearly where they should stop and speak to the
   clinician rather than rely on this sheet
