The Incident & Adverse Event Report

Help me write a factual incident report.

Type of event: [DESCRIBE CATEGORY, NOT PATIENT DETAIL]
Setting: [DESCRIBE]

Produce a report that:
1. States facts in chronological order, timestamped where I have
   given times
2. Separates what was observed from what was inferred — clearly
   labelled
3. Uses neutral language throughout — no blame, no speculation
   about cause, no characterisation of anyone's competence
4. Records actions taken in response
5. Records who was notified and when
6. Identifies contributing factors as questions for investigation
   rather than as conclusions

- Do not add facts
- Do not editorialise
- Where my account is ambiguous, flag it rather than resolving it

This is a factual report for the incident system, not a defence
document.
